The Export function exports all the meta data so that it could be shared across different applications. However, if you want the data in row/per record, here is the process to transpose the data.
Attached is a file of 3 example records that were exported. The first sheet was renamed "original" from "hipaa_form_bulk_forms_export". The second "Transposed" sheet was added.
On the Transposed sheet, you can use the formulas in the example. For the labels, in Cell A1 formula change the 16 to the number of rows/records-1. Then copy across the number of rows.
In Cell A2 formula change the 17 to number of rows/record. That will be copied across them number of fields and down the number of records.
