One of the causes for this issue is using the Default and NOT having a "Send Notifications to:" In this case, the plugin will default to the email defined in Gravity notifications. However, the only option that works for this scenario is having the "Send To" field with "Enter Email" The other 2 options will not work. With this option, the email in the "Send to Email" will be notified.
