There are significant changes with Wordpress V5.5 with regards to jQuery. There may be plugins that you are using that are causing jQuery issues "above" the HIPAA Forms Plugin that will cause the HIPAA forms Plugin not to function correctly on the front-end, the back-end or both.

Within the article, you will see a link to the this plugin : Test jQuery Updates. We installed and tested in our V5.5 Environments to make sure that HIPAA Forms is working.
This issue is going to continue in the future. Our recommendation at this point is to go through your current plugins and disable them one at a time. At some point, you will see HIPAA Forms work correctly.
